A speeding Wal-Mart truck driver who had been awake for 28 hours was held responsible on Tuesday for a 2014 highway crash that left “30 Rock” star Tracy Morgan with brain injuries and killed a fellow comedian, U.S. transport regulators said.

This morning we discussed a new report related to the Tracy Morgan auto crash, which was caused by a trucker who rear-ended Morgan’s limo while speeding in a construction zone.

The NTSB released the probable cause and made nine new safety recommendations after a Tuesday morning board meeting in Washington, finishing months of investigation.

The crash has similarities with other truck accidents and raises safety concerns about how these large vehicles are operated in work zones, according to the NTSB.
